Imaging in the evaluation and follow-up of early and advanced breast cancer: When, why, and how often?

Asymptomatic patients with Stage 0-II breast cancer (BC) should not have imaging scans to screen for metastatic disease. Patients with Stage III BC should have advanced imaging (bone scan, CT or PET) at diagnosis to screen for metastatic disease. In Stage 0-III BC treated for cure, the goal of surveillance imaging is to detect a recurrence or a new primary cancer. After lumpectomy for Stage 0-III BC, surveillance includes imaging the affected breast and the intact contralateral breast. After bilateral mastectomies for Stage 0-III BC, women should not receive breast/chest wall imaging for surveillance. The goal of imaging in metastatic BC is to guide treatment choices that will maximize both length and quality of life.
