In patients who received a pacemaker, R-on-T was often related to ventricular undersensing problems or asynchronous pacing. DDD pacing mode permitted post-atrial ventricular blanking, with potential undersensing of the native QRS complex. Patients with a permanent dual chamber pacemaker and atrial undersensing are exposed to potential R-on-T risk.
