Radical prostatectomy improves overall- and cancer-specific survival compared to observation alone, particularly in younger men. Radical prostatectomy also reduces the risk of metastatic disease and need for hormone therapy. No randomized studies have evaluated contemporary radiation modalities and surgery. Well known biases frequently confound the interpretation of observational comparative-effectiveness studies between surgery and radiation. More long-term clinical data is necessary for cryoablation and HIFU before widespread implementation of these modalities in the primary treatment of localized prostate cancer.
