We investigated the use of frozen sections (FSs) at radical prostatectomy (RP).
Sensitivity and specificity were 85% and 100% for diagnosis of adenocarcinoma.
Positive FS was associated with perineural invasion on biopsy and higher stage at RP.
Accuracy of FS in predicting the overall prostatectomy final margin status was 81%.
We suggest that FS has limited, if any predictive utility, for final margin status.
