A three compartment model of glucose–insulin kinetics in the very premature infant is presented. Insulin kinetic parameters are identified using a novel clinical data set, alongside literature data. Kidney function and extracellular fluid volumes are tailored by infant age and weight. The final model shows similar or improved ability to capture clinical data over previous model iterations, and improved glycaemic control performance in use. The final model is more physiologically comprehensive, but is still identifiable for bedside use in glycaemic control.
