Scarf osteotomy is a versatile procedure for hallux valgus correction. Shortening scarf osteotomy is increasingly being used to correct severe deformity. There has been concern that shortening scarf osteotomy may result in transfer metatarsalgia. In our series when the metatarsal is shortened, patients did not develop transfer metatarsalgia at a mean follow-up of 25 months. Shortening scarf osteotomy results in excellent outcome and high patient satisfaction.
