Recent times have seen increasing frequencies of mucormycosis, especially in immunosuppressed individuals. This infection is associated with high mortality. Clinical manifestations of the disease vary, but rhinoorbitocerebral and pulmonary involvements are more common. Here we present a case of a 32-year-old lady who was admitted with acute febrile illness with hemophagocytic syndrome. She was successfully treated but later developed an isolated hepatic mucormycosis with a rapid fatal course. This case highlights the importance of a high index of clinical suspicion for fungal infections especially in an immunosuppressed state. It emphasizes the need for aggressive and extensive work up to diagnose infections at unusual sites.
