In the case of the death of a 26-year-old, police-known drug abuser in a penitentiary, the finding situation and preliminary results of the criminal investigation pointed to drug intoxication as the cause of death. However, the autopsy and further systematic investigations showed a natural death due to desquamative interstitial pneumonia. This disease is among the rare diseases that need to be kept in mind in medicolegal practice. The presented case illustrates the importance of autopsies and follow-up investigations, in particular, that of histological investigations.
