The correction of severe malocclusions is an interdisciplinary task for orthodontic and maxillofacial surgeons. The aim is a three-dimensional normalization of the dentition and a permanent stabilization. Particularly severe malocclusions with three-dimensional characteristics are currently often corrected in a 2-sided manner after a 12-month interval. If the complete facial skeleton and the surrounding soft tissues are also included in the planning, new aesthetic aspects must be considered which often lead to substantial complex skeletal displacement of the middle and lower facial bones bearing the soft tissue contour and function in all three dimensions in mind. Prerequisites for aesthetic operative results are extensive planning of the surgical approach and exact operative realization. Correction of malocclusions has become a routine procedure and is nowadays only a part of the comprehensive construction of the middle and lower facial structures. The present article describes all aspects of the aesthetics of orthognathic surgery.
