The German healthcare system is unique with respect to its structure and organization when compared internationally as it is a self-governed system. The German government only determines the regulatory policies. The individual federal states are responsible for ensuring adequate inpatient hospital services for the population. For this purpose, state-specific hospital plans are established and state-specific hospital laws are issued. The public mandate of individual hospitals is the basis for the extent of healthcare services provided. Essential actors in self-governance are corporations and associations, for example state-specific organizations of the benefactors and the providers. The partners in self-governance do not always have uniform interests and compromises are often necessary. Provision of services within the German healthcare system is structured in sectors, e.g. inpatient and outpatient sectors, which are financed via separate structures. Especially at the intersections of sectors, contrasting interests may arise between healthcare providers. The statutory health insurance funds are independent with respect to financing and organization and self-responsibly pursue the duties conferred upon them by the state. The core principles of the statutory health insurance funds are the principles of solidarity and benefits in kind. For all actors in the German healthcare system the principle of efficiency has priority.
