Because of the close anatomical proximity, organ injuries during gynecological pelvic surgery cannot always be prevented. The adoption of minimally invasive techniques has resulted in great improvements for patients but at the same time was associated with new complications, which were previously unknown in this form. The complexity of endoscopic techniques has increased after oncological procedures were also included. Complication rates remain low with adequate expertise but the possible morbidity means that particular attention must be given to the informed consent (§630 German Civil Code, Patient Rights Act) taking into account that treatment of organ injuries has an extensive complication rate and can significantly reduce the long-term quality of life. Due to the low number of recognized intraoperative injuries, the close proximity of the organs should be given high priority during surgery and the high number of thermal lesions due to the use of electrocoagulation and dissection should act as a reminder for careful application.
