Hip pain as the first symptom of acute promyelocytic leukemia: a case report of a pelvic chloroma

Granulocytic sarcoma (GS), also known as chloroma, is characterized by the presence of malignant myeloid blast cells in tissues outside the hemopoietic system. It occurs usually in patients with acute myeloid leukemia and commonly affects orbital bones, ribs, and sternum. It rarely involves the pelvis or the extremities. This fact, added to the practical absence of need for surgery in this location makes GS an almost unknown tumor for the general orthopedic surgeon. We report a case of refractory pain of the hip of a 22-year-old patient caused by a GS in the context of acute leukemia. Magnetic resonance appearance can establish the diagnosis of GS without the need of a biopsy in this appropriate context of leukemic manifestation.
