The safety of drug use during pregnancy and when planning a pregnancy is a territory of uncertainty. Brief information in the “Rote Liste” or in package leaflets often suggests that drugs should not be used during pregnancy. Although the majority of currently used drugs have not been systematically evaluated, pregnant women or those planning a pregnancy still need to be adequately treated. Consequently, medicinal therapy during pregnancy is often off-label. This article provides recommendations for acceptable drugs and gives an overview on the current state of knowledge on analgesics, antibiotics, antihypertensives, coumarin anticoagulants, immunomodulating drugs, antirheumatics, retinoids, antiepileptics and psychotropic drugs. Many pregnancies are unplanned; therefore, drug treatment during the reproductive age should take the possibility of pregnancy into consideration. Whenever feasible, drugs with proven efficacy and compatibility for the unborn child should be preferred.
