Treatment with a mandibular advancement splint (MAS) is preceded by staging diagnostics in sleep medicine and an interdisciplinary cooperation. Determination of the diagnosis of obstructive sleep apnea syndrome (OSAS) and the degree of severity give rise to the indications for therapy and the available options. Adjustable two-splint systems are evidence-based therapeutic procedures with a high compliance. Their application must take local and general circumstances into consideration and necessitate imaging examinations as well as adherence to the manufacturers’ technical principles. A temporary test splint and accompanying polygraphic screening can help to classify the MAS to the therapy options and if necessary open up the options for mandibulomaxillary advancement in younger patients. Adjustability, compliance and the side effect profile of splint treatment necessitate continuous monitoring. Under socioeconomic aspects the MAS is more cost-effective than continuous positive airway pressure (CPAP) therapy.
