The purpose of this study was to identify specific performance validity tests (PVTs) or standard neurocognitive tests and/or combinations of tests differentiated credible patients with psychosis against non-credible patients feigning psychosis. Group comparisons were computed on archival neuropsychological test data from 57 credible patients and 53 non-credible patients. Overall, the credible group outperformed the non-credible group on most tests, but groups did not differ on several executive scores, false positive errors on recognition memory tasks, processing speed tasks, and a visual memory task. All original PVT cutoff scores had to be adjusted to maintain adequate specificity in the credible sample, with the exception of the RAVLT/RO discriminant function and time to recite 3- and 4-digits forward. Tabulation of failure rates of 10 most sensitive (i.e., > 47% sensitivity) scores in the two groups separately revealed that ≥3 failures was associated with 91.3% specificity and 83.8% sensitivity. The method described in the current investigation shows how PVT scores can be used in combination to effectively discriminant credible and non-credible groups while successfully limiting false positive identifications.
