A less well known method of performing EMG of genioglossus in children is to approach the muscle subcutaneously from under the chin. This is well tolerated by children of any age and offers an opportunity of readily examining the bulbar musculature. It is indicated in isolated bulbar problems or to confirm the widespread nature of anterior horn cell disease with important implications for prognosis and diagnosis. Twenty-three children were studied using this technique. Twelve showed abnormalities, all except two indicating a neurogenic abnormality. In 3 of the 9 patients with isolated bulbar palsy showing EMG abnormality, this was the only muscle affected. Wider appreciation of this technique may assist the diagnosis of neuromuscular disease in children.
