De Parscau Abstract Les hernies inguinales sont fréquentes chez les enfants (entre 1 et 13%). L’étranglement herniaire se manifeste par une tuméfaction inguinale, dure, douloureuse et irréductible et impose un traitement chirurgical en urgence. Sa fréquence est estimée entre 1,5 et 8%. L’ovaire s’extériorise dans le canal de Nück chez le nourrisson de sexe féminin et l’intestin grêle chez la petite fille. L’étranglement herniaire est plus exceptionnel que chez le garçon. Cependant la cure chirurgicale reste de mise dans les deux cas. Les auteurs rapportent l’observation d’une petite fille âgée de 6 ans, admise aux Urgences Pédiatriques pour une tuméfaction inguinale, douloureuse, apparue brutalement quelques heures auparavant. Il existait une masse oblongue s’étendant de l’orifice inguinal superficiel à la grande lèvre gauche, douloureuse à la palpation, irréductible. Le reste de l’examen clinique était normal avec en particulier l’absence designes cliniques d’occlusion. Les examens complémentaires ont comporté un cliché d’abdomen sans préparation à la recherche d’un niveau hydro aérique pelvien (absent) et une échographie. En effet, la bonne tolérance clinique en dehors des manipulations, était discordante avec le diagnostic d’hernie étranglée. L’examen échographique mettait en évidence une image hypo-échogène, liquidienne, allant de l’orifice inguinal profond à la grande lèvre. Dans le contexte, l’hypothèse d’une hernie inguinale étranglée ne pouvant être formellement éliminée, l’intervention chirurgicale a été réalisée en urgence. Cette dernière a retrouvé un lymphangiome kystique indépendant du canal de Nück dont l’exérèse chirurgicale a été réalisée en un temps. Le recul est de 1 an. Il n’y a eu aucune récidive. Les tuméfactions du canal inguinal sont fréquentes chez l’enfant et dominées par les hernies inguinales. L’examen clinique permet de faire le diagnostic de certitude dans la majorité des cas. Cependant, il existe quelques diagnostics différentiels rares qui peuvent en imposer pour des étranglements herniaires comme dans cette observation. Purchase PDF (78 K) |
