Introduction: Most pituitary macroadenomas with large and giant midline symmetrical suprasellar extension (SSE) can be successfully removed via the transsphenoidal approach. In some cases however, it is impossible to obtain the detachment of the SSE from the diaphragm sellae and further manipulation may be deleterious to optic structures, carotid arteries or hypothalamus. Thus a subtotal removal is carried out at first approach. The second session is planned deliberately as a gradual downward progression of the SSE into the sella on sequential post-operative imaging studies is observed. In such cases, a second operation is done via the same transsphenoidal route, thus avoiding a transcranial procedure.
