摘要
GLUT-1 protein is the principal glucose transporter across the blood-brain barrier. GLUT-1 deficiciency results in a syndrome of infantile seizures refractory to anticonvulsive drugs, developmental delay, acquired microcephaly and neurologic manifestations including spasticity, hypotonia, and ataxia. A low cerebrospinal fluid glucose concentration in the absence of hypoglycaemia is pathognomonic of glucose transporter deficiency syndrome. Ketogenic diet is an effective treatment of epileptic manifestations but it has less effect on the cognitive symptoms. We report on a child who presented with paroxistical events often occurring prior to meals, developmental delay, microcephaly and spasticity. CSF and serum glucose levels measured simultaneously showed a CSF/serum glucose ratio of 0.39. Molecular analysis identified a heterozygous novel mutation.

Abstract

le="line-height:150%">The authors have studied the response to hypercapnia and hypocapnic hypoxia in nine dogs anesthetised with Althesin, in correlation with three levels of anaesthesia defined by three different anaesthetic flow rates (A = 6.57 ± 2.00 μl . kg−1 . min−1; B = 13.88 ± 2.87 μl . kg−1 . min−1; C = 19.53 ± 5.34 μl . kg−1 . min−1). The animals were intubated. Ventilation was measured by means of a pneumotachograph. Arterial blood gases (pHa, PaCO2, PaO2) were measured before and at the end of each hypercapnia and hypocapnic hypoxia test. The results were compared with data in the literature concerning the dog awake. Increasing depth of anaesthesia leads to worsening hypoventilation and hypercapnia. As with other anaesthetic agents, the response to hypercapnia is more depressed the deeper the anaesthesia. On the other hand, unlike what has been described with other anaesthetic agents, hypoxia stimulates ventilation; the response to hypoxia is not abolished by the deeper states of anaesthesia. The possible mechanisms of this difference are discussed.

Les auteurs ont étudié les réponses à l'hyper capnie et à l'hypoxie hypocapnique de neuf chiens anesthésiés avec de l'Alfatésine en fonction de trois niveaux d'anesthésie déterminés par les débits différents de l'anesthésique (A = 6,57 ± 2,00 μl . kg−1 . min−1; B = 13,88 ± 2,87 μl . kg−1 . min−1; C = 19,53 ± 5,34 μl . kg−1 . min−1). Les animaux ont été intubés. La ventilation a été mesurée à l'aide d'un pneumotachographe. Les gaz du sang artériel (pHa, PaCO2, PaO2) ont été mesurés avant et à la fin de chaque test d'hypercapnie et d'hypoxie hypocapnique. Les résultats observés ont été comparés à des données de la littérature se rapportant au chien éveillé. L'approfondissement de l'anesthésie entraîne une hypoventilation et une hypercapnie croissantes. Comme avec les autres agents anesthésiques la réponse à l'hypercapnie est d'autant plus déprimée que l'anesthésie est plus profonde. Contrairement à ce qui a été décrit avec d'autres agents anesthésiques, l'hypoxie stimule la ventilation; la réponse à l'hypoxie n'est pas abolie même à des stades profonds de l'anesthésie. Les mécanismes possibles de cette différence sont discutés.
