基于代谢组学的中药“红I号”灌肠液对盆腔炎性疾病后遗症(寒湿瘀滞证)的作用研究

摘要
目的:通过代谢组学研究中药"红I号"灌肠液对盆腔炎性疾病后遗症(寒湿瘀滞证)的作用。方法:符合纳入标准的寒湿瘀滞证盆腔炎性疾病后遗症(SPID)患者28例直肠给药,进行治疗前后的比较;另选择5例健康人为健康对照组。初步探索中药"红I号"灌肠液通过直肠给药方式治疗盆腔炎性疾病后遗症(寒湿瘀滞证)的安全性和有效性。同时收集盆腔炎性疾病后遗症(寒湿瘀滞证)患者的粪便样本,共设5组,分别于第1天、第5天、第10天、第15天、第20天抽提并获得粪便菌群DNA。用代谢组学方法筛选出关键的生物标志物或与疾病有一定相关性的标志物,从而分析药物的吸收功能、代谢功能以及生物利用度等的影响。结论:寒湿瘀滞证SPID患者粪便代谢物中存在有差异的关键化合物,其中寒湿瘀滞证SPID患者与正常健康人粪便中潜在干预靶点比较,4个上调,4个下调。这些代谢化合物的异常表达可能是SPID的重要发病机制。
        Objective: To observe the effect of TCM "Red I" Enema on sequelae of pelvic inflammatory diseases(SPID) of cold dampness and stagnation based on metabolomics study. Methods: A total of 28 patients met the inclusion criteria were treated with TCM "Red I" Enema, and the curative effect was compared before and after the treatment. Another five healthy people were selected as the blank control. The safety and effectiveness of TCM "Red I" enema in treating SPID(syndrome of cold dampness and stagnation) by means of rectal administration were preliminarily explored. At the same time, fecal samples of patients were collected and the fecal flora DNA was extracted on day 1, day 5, day 10, day 15 and day 20 respectively. Metabolomics was used to screen out the key biomarkers or those correlated to the diseases, so as to analyze the effects of absorption function, metabolic function and bioavailability of the enema. Results: There were key different compounds in fecal metabolites in SPID patients of cold dampness and stagnation, among which, four was up-regulated and four was down-regulated in terms of potential intervention targets in feces of SPID patients. Abnormal expressions of these metabolic compounds may be an important pathogenesis of SPID.
