摘要
目的:通过门诊药房信息化药品调剂差错防范体系的建立与运行,有效降低药品调剂差错率,提高药学服务质量,保障患者安全用药。方法:从药品调剂前、中、后3个维度分别展开,运用信息化技术,设计并建立"医师-药师-患者"三位一体的药品调剂差错防范体系,并对该体系运行前后效果进行分析比较。结果:门诊药房信息化药品调剂差错防范体系建立运行后,药品调剂工作的各环节差错均显著减少,调剂前医师开方错误率从0.724 4%下降为0.340 3%;调剂中药师总调配差错率从0.078 8%下降为0.042 4%,其中各调配差错类别的占比也均有所下降;调剂后出门差错率从0.000 68%下降为0.000 26%。结论:门诊药房信息化药品调剂差错防范体系的建立,为减少药品调剂差错提供了有效的新方法,使药师充分发挥专业技术水平,为患者提供优质的信息化药学服务,保障患者安全用药,是未来医院药房降低药品调剂差错率的发展趋势。
        OBJECTIVE To establish and operate the informatization drug dispensing error prevention system to effectively reduce dispensing errors in outpatient pharmacy,improve the quality of pharmaceutical care and ensure safe medication.METHODS Investigatation and analysis were performed from three aspects:before,during,and after dispensing.Information technology was adopted to design and establish a dispensing error prevention system integrating "physicians,pharmacists,patients".Reduction of dispensing errors were compared before and after the system was established.RESULTS The operation of informatization drug dispensing error prevention system in outpatient pharmacy significantly reduced dispensing errors,from0.724 4% to 0.340 3% before dispensing,and from0.078 8%to 0.042 4% during dispensing.Proportions of different dispensing error categories also declined.Error rate after dispensing was declined from0.000 68%to 0.000 26%.CONCLUSION Informatization drug dispensing error prevention system provides an effective method to reduce dispensing errors,fully utilize the professional technique of pharmacists,provide high quality informatization pharmaceutical care for patients,and ensure safe medication.Informatization drug dispensing error prevention system is development trend in future for hospital pharmacies to reduce dispensing error rate.
