乳腺钼靶定位对成簇钙化灶行细针穿刺活检细胞学检查的临床研究

摘要
目的:探讨乳腺钼靶定位对成簇钙化灶行细针穿刺活检细胞学检查的临床研究。方法:选自我院于2015年12月—2017年12月期间收治临床初诊阴性的乳腺疾病患者83例,所有患者经钼靶X线检查示乳腺实质内有单纯成簇钙化灶,无其他直接影像学征象。所有研究对象均行细针穿刺细胞学检查和乳腺钼靶定位。结果:经细针穿刺诊断,良性43例、可疑2例、恶性38例。经术后病理诊断,良性44例、恶性39例。恶性乳腺疾病患者簇状钙化个数> 1个多于良性乳腺疾病患者,钙化点数目≥15个/cm~2多于良性乳腺疾病患者,钙化灶直径≥10mm多于良性乳腺疾病患者,簇状钙化密度低少于良性乳腺疾病患者,具有统计学差异(P <0.05)。结论:乳腺钼靶定位对成簇钙化灶行细针穿刺活检细胞学检查具有重要意义,其在临床触诊阴性乳腺癌早期诊断具有重要价值,值得临床借鉴。
Objective To investigate the clinical significance of mammography in fine needle aspiration cytology for clustered calcifications. Methods From December 2015 to December 2017, 83 cases of breast disease with negative clinical diagnosis were treated in our hospital. All patients showed simple cluster calcification in the parenchyma of the breast by molybdenum target X-ray examination. All the subjects underwent fine needle aspiration cytology and mammography. Results By fine needle aspiration, 43 cases were benign, 2 were suspicious and 38 were malignant. According to postoperative pathological diagnosis, 44 cases were benign and 39 cases were malignant. The number of calcified calcification in patients with malignant breast disease was more than 1 more than those with benign breast disease. The number of calcification points more than 15/cm2 were more than those of benign breast disease. The diameter of calcification was more than 10 mm more than those of benign breast disease, and the cluster like calcification density was lower than those of benign breast disease(P < 0.05). Conclusion Mammary mammography is of great significance for the fine needle biopsy cytology of cluster calcification. It is of great value in the early diagnosis of clinical palpation negative breast cancer, and it is worthy of clinical reference.
