摘要
目的:计算机辅助下个性化诊疗颌骨肿瘤,提高诊断的准确性、手术的精确性,缩短了手术时间。方法:颌骨肿瘤病例40例,分为两组,每组20例。A组在计算机辅助下个性化诊疗,B组经三维重建得到的立体模型,按以往经验,术中切除肿瘤,常规取骨或重建板重建下颌骨。对比两组病例术前、术后三维CT重建效果,从冠状面、矢状面、横断面对比各个标记点较术前移位情况,手术时间,肿瘤复发率。结果:所有患者均顺利完成手术,术后三维CT重建显示A组重建的颌骨外形良好,与术前设计基本一致,颌骨标记点较术前平均移位(1.53±0.17)mm,随访2年均无复发。B组大部分颌骨外形良好,颌骨标记点较术前平均移位(3.53±0.57)mm,随访2年有2例复发。结论:计算机辅助下个性化诊疗颌骨肿瘤,可提高诊断的准确性及修复效果,缩短了手术时间。
        Objective:The applications o of the computer-assisted personalized therapy of maxillofacial bone tumor,may improve the accuracy of diagnosis and surgical precision and reduce surgery time. Method:40 patients with maxillofacial bone tumor were collected,and were randomly divided into A,B two group. Patients in group A were treated with computer-assisted personalized therapy,the three-dimensional model was obtained after reconstruction of group B patients,according to past experience,remove the tumor and reconstructed mandible. Then 3D CT reconstructions were performed in every patient The assessments of maxillofacial markers were gained from coronal,sagittal,cross-sectional and the surgery time and the tumor recurrence rate. Result:All patients were successfully operated,three-dimensional CT reconstruction displaying all patients in group A reconstruction of the mandible in wonderful shape,consistent with the preoperative design,maxillofacial markers compared with preoperative average shift(1.53±0.17)mm,all patients have been follow-up 2 years,there is no relapse. Group B patients in good shape most of the mandible,maxillofacial markers compared with preoperative mean shift(3.53±0.57)mm,all patients have been follow-up 2 years,2 cases recurring. Conclusion:computer-assisted personalized therapy of maxillofacial bone tumor can effectively improves the accuracy of diagnosis,improved repair effect,reduce the operation time.
