摘要
目的研究多层螺旋CT(MSCT)定量研究技术参数在肺癌患者中的表现及其与预后的关系。方法回顾性分析2012年1月~2018年10月我院收治80例肺癌患者的临床资料和随访资料,对其MSCT检查结果和肺功能仪检查结果进行整理和比较,纳入同期于我院行体检的健康人50例为正常对照组,将其检查结果与肺癌患者进行比较和相关性分析,对80例肺癌患者进行随访调查,根据其预后情况将其分为预后良好组和预后不佳组,对比两组患者MSCT参数。结果肺癌患者vex、vin-vex、vex/vin、 MLDex、MLDexMLDin、单位体积密度与正常人比较,差异有统计学意义(P <0.05);肺癌患者MSCT参数与大部分肺功能仪检查参数均具有良好相关性,Vin与TLC、Vex与RV、vin-vex与FVC、vin/vex与RV/TLC相关性最佳,均呈正相关(r=0.75、0.84、0.79、0.91,P <0.01),EVin与FEV1、EVex与FEV1相关性较佳,均呈负相关(r=-0.56、-0.78,P <0.01);预后良好组患者Vin、vex、vin-vex、vex/vin与预后不良组患者比较,差异均有统计学意义(P <0.05)。结论 MSCT定量检查肺癌患者肺功能与肺功能试验相较更为准确、客观,其定量参数与肺功能存在良好相关性,可提示肺癌患者预后。
        Objective To study the performance of multi-slice spiral CT(MSCT) quantitative research technique parameters in patients with lung cancer and its relationship with prognosis. Methods The clinical data and followup data of 80 patients with lung cancer who were admitted to our hospital from January 2012 to October 2018 were retrospectively analyzed. The results of MSCT examination and pulmonary function instrument were collated and compared.50 healthy people who underwent physical examinations in our hospital during the same period were selected as the normal control group. Results of examination of those healthy people were given comparison and correlation analysis with those of patients with lung cancer. 80 patients with lung cancer were followed up for investigation. They were divided into good prognosis group and poor prognosis group according to the prognosis, and the MSCT parameters were compared between the two groups. Results There were statistically significant differences in the vex, vin-vex, vex/vin, MLDex, MLDex-MLD and unit volume density between patients with lung cancer and normal people(P < 0.05). The MSCT parameters of patients with lung cancer had good correlation with most parameters of pulmonary function instrument. Vin and TLC, Vex and RV, vinvex and FVC, vin/vex and RV/TLC had the best correlation and they were positively correlated(correlation coefficients were0.75, 0.84, 0.79, 0.91, P < 0.01). And EVin and FEV1, EVex and FEV1 had good correlation, and they were negatively correlated(correlation coefficients were-0.56,-0.78, P < 0.01). There were statistically significant differences in Vin, vex,vin-vex and vex/vin between good prognosis group and poor prognosis group(P < 0.05). Conclusion MSCT quantitative examination of lung function in patients with lung cancer is more accurate and objective than pulmonary function instrument.And its quantitative parameters have good correlation with lung function, which can indicate the prognosis of patients with lung cancer.
