某院静脉用药调配中心不合理医嘱分析

摘要
目的分析某院静脉用药调配中心不合理用药医嘱,为临床安全、合理用药提供参考。方法对某院2017年7月～2018年6月各病区静脉用药医嘱进行合理性审查,对发现的不合理医嘱进行统计分析,并分析干预前后不合理医嘱的发生情况。结果共有1068条不合理医嘱,占总医嘱数的1.79%,类型主要包括溶媒选择、溶媒量、给药剂量、给药频次、配伍禁忌、禁忌证、医嘱下达错误、其他等,干预后不合理医嘱发生率低于干预前(P<0.01)。结论对不合理医嘱进行定期分析及干预,可降低静脉用药调配中心不合理医嘱发生率,提高静脉用药合理性。
Objective To analyze the irrational medical advices in the pharmacy intravenous admixture services(PIVAS) in our hospital, to provide references for clinical safe and rational drug use. Methods The medical advices of PIVAS in our hospital were checked from July 2017 to June 2018.The irrational medical advices were summarized and analyzed statistically, which were also comparatively analyzed before and after intervention.Results A total of 1068 irrational medical advices were reviewed, accounting for 1.79% of the total medical advices. The types of the irrational medical advices contained incorrect selection of solvent, amount of solvent,improper dosage, improper frequency, incompatibility, contraindication, man-made entry errors of drug information and so on. After intervention,the incidence rate of irrational medical advices were lower than previously(P<0.05).Conclusion The periodic analysis and intervention can reduce the incidence rate of irrational medical advices,and improve the rationality of intravenous drug use.
