乳酸菌阴道胶囊联合雌激素治疗对老年阴道炎病人炎症因子和阴道微生态的影响

摘要
目的探讨乳酸菌阴道胶囊联合雌激素治疗对老年阴道炎病人炎症因子和阴道微生态的影响。方法选取2015年12月至2017年9月于海口市三家医院诊治的120例老年阴道炎病人为研究对象,根据随机数字表法将病人分为研究组和对照组,每组各60例。对照组采用雌三醇软膏治疗,观察组采用雌三醇软膏联合乳酸菌阴道胶囊治疗。比较2组病人治疗前后血清炎症因子白细胞介素-6(IL-6)、肿瘤坏死因子α(TNF-α)、超敏C反应蛋白(hs-CRP)水平,pH值、症状评分及乳酸菌阳性率、阴道健康评分的变化及临床疗效。结果与治疗前相比,治疗后2组病人IL-6、TNF-α、hs-CRP水平,pH值,阴道瘙痒感、疼痛感、阴道分泌物评分均明显降低,乳酸菌阳性率和阴道健康评分显著升高(均P<0. 05),且研究组改善更显著(均P<0. 05)。研究组有效率为95. 00%,显著高于对照组的78. 33%(P<0. 05)。结论乳酸菌阴道胶囊联合雌激素治疗可有效治疗老年阴道炎,缓解炎症应激反应,改善阴道炎临床症状和阴道微生态环境,值得临床推广。
Objective To explore the effect of lacidophilin vaginal capsules combined with estrogen therapy on inflammatory factors and vaginal microecology in the elderly patients with vaginitis.Methods A total of 120 elderly patients with vaginitis treated in three hospitals of Haikou from December 2015 to September 2017 were selected in this study. Patients were divided into study group and control group according to the random number table method,with 60 cases in each group. The control group was treated with estriol cream. The observation group was treated with estriol ointment and lacidophilin vaginal capsules. The clinical efficacy and the levels of serum inflammatory cytokines including interleukin 6( IL-6),tumer necrosis factor α( TNF-α),high sensitivity C reactive protein( hsCRP),and pH,symptom scores,positive rate of lactic acid bacteria,vaginal health score were compared between the two groups before and after treatment. Results After treatment,the levels of IL-6,TNF-α,hs-CRP,pH,the degree of vaginal itching,pain,and vaginal secretion scores of the two groups were significantly reduced, and the positive rate of lactic acid bacteria and vaginal health scores were significantly increased( all P<0. 05),and especially in the study group( all P<0. 05). The effective rate was 95. 00% in the study group,which was significantly higher than that in the control group( 78. 33%)( P<0. 05). Conclusions Lacidophilin vaginal capsules combined with estrogen therapy is an effective treatment method for senile vaginitis,which can relieve inflammatory stress response,improve vaginal clinical symptoms and micro-ecological environment,and is worthy of clinical promotion.
