ICL V4c治疗高度近视眼双通道系统视觉质量分析

摘要
目的:应用双通道视觉质量分析系统评价V4c型可植入式隐形眼镜(implantable contact lens V4c,ICL V4c)植入术矫正高度近视眼的术后视觉质量及有效性。方法:采用前瞻性系列病例研究。选取近视度数为-6.00～-12.00D患者30例60眼,年龄18～35岁,术前最佳矫正视力≥0.6,角膜散光≤1.0D,全部采取颞侧透明角膜切口,植入ICL V4c后房型人工晶状体,分别在术后1wk,1、3mo进行随访观察,检查项目:术前最佳矫正视力(BCVA)、术后裸眼视力(UCVA)、术眼的OSI(客观散射指数)、MTF cut off(调制传递函数截止频率)、SR(斯特列尔比值)和100%、20%及9%的OV值(对比度视力)。应用SPSS19.0统计学软件进行数据处理,手术前后各项参数比较采用重复测量数据的方差分析。结果:术后1wk,1、3mo的UCVA逐渐改善,与术前BCVA比较均有统计学意义(P<0.01)。术后1wk,1、3mo的OSI、MTF cut off、SR、OV 100%、OV 20%及OV 9%均优于术前,与术前比较均有统计学意义(P<0.05)。结论:ICL V4c植入术能有效矫正-6.00～-12.00D的高度近视,术后OSI比术前减小,术后UCVA优于术前BCVA,术后的MTF cut off、SR、100%、20%、9%的OV值比术前均有提高。
AIM :To evaluate the effectiveness of visian implantable collamer lens with central hole(ICL V4 c) implantation for the correction of high myopia using the double-pass visual quality analysis system.· METHODS: Totally 60 eyes of 30 high myopia patients who underwent ICL V4 c implantation were enrolled into this prospective randomized control study. Myopic degree of selected patients was-6. 0 D to-12. 0 D, age 18 to 35 years old, best corrected visual acuity(BCVA)≥ 0. 6,preoperative corneal astigmatism acuities,≤1. 0 D. All patients were taken the temporal side transparent corneal incision. The patients were evaluated postoperatively1 wk, 1 and 3 mo respectively. The evaluating items included preoperative BCVA, postoperative uncorrected visual acuity(UCVA), objective scattering index(OSI),and modulation transfer function( MTF) cut off frequency, Strehl ratio(SR), and 100%, 20%, 9% Optical Quality Analysis System(OQAS) Value(OV,contrast visual acuity). All statistical analyses were performed with SPSS19. 0 statistical software. Variance analysis of repeated measurement data was employed.· RESULTS: The UCVA gradually improved at 1 wk, 1 and3 mo after surgery. There was statistical significance compared with the preoperative BCVA( P < 0. 01). The postoperative OSI, MTF cut off, SR, OV 100%, OV 20% and OV 9% at 1 wk, 1 and 3 mo showed statistically differences compared with the preoperative values(P<0.05).· CONCLUSION: ICL V4 c implantation can effectively correct high myopia range from-6. 0 D to-12. 0 D. The postoperative OSI were smaller than the preoperative OSI. The postoperative UCVA were better than the preoperative BCVA. The value of postoperative MTF cut off,SR,and OV value of 100%,20% and 9% improved.
