中国医疗保障70年:回顾与解析

摘要
中华人民共和国成立以来,医疗保障制度经过了从无到有、试点探索、全民覆盖、新一轮完善等发展阶段。医保制度70年的演变脉络表现为:制度架构从多元分割到逐步整合,保障体系从单一到多层次,保障责任从个人缺位到多方分担,保障对象从部分到全民普惠,管理体制从集体管理到社会化管理。纵观我国医保制度70年的历史演进,一方面,预防为主、注重初级卫生保健、强调全民参与,最终实现卫生领域的低投入与高产出是计划经济时期的突出成就;另一方面,经济体制、卫生服务体系与医疗保障制度之间的协同发展构成了整个医疗保障变迁的内在规律。
Since the founding of the People's Republic of China, its healthcare system has undergone several stages of development: starting from scratch to pilots to universal coverage and new improvement. The shifts in the healthcare system in the past 70 years include the following: the segmented schemes for diff erent groups of people were gradually integrated; the single tier protection system developed into a multi-tier one; the insured individuals joined the governments and enterprises to share responsibilities; the partial coverage was replaced with the universal coverage; the management and operation was handed over to professional agencies. Throughout the 70-year development, the low-input and high-output in the health sector, which was achieved by prioritizing prevention, primary healthcare and involvement of all people, was a prominent achievement in the planned economy period. Separately, the coordinated development of the economic system, health services and health insurance constitutes the inherent law of the transformation of the healthcare system.
