喉罩全麻在老年高血压患者行腹腔镜胆囊切除术中的应用效果观察

摘要
目的探讨高血压老年腹腔镜胆囊切除术中采用喉罩全麻的临床效果与安全性。方法以2016年1月—2018年12月为时限,择取该院52例合并高血压的老年腹腔镜胆囊切除手术患者,随机分为两组。实验组26例,术中予以喉罩全麻;对照组26例,术中予以气管插管全麻。比较观察两组术中生命体征,统计术后通气相关并发症及患者麻醉体验满意度。结果对照组术中T_1、T_4时刻HR、SBP、DBP波动较明显,实验组相对平稳。实验组术后通气相关并发症发发生率(7.69%)低于对照组(26.92%),麻醉体验满意度(96.15%)高于对照组(80.77%),差异有统计学意义(χ~2=8.316,P<0.05;χ~2=5.739,P<0.05)。结论老年高血压患者腹腔镜胆囊切除术中插喉罩全麻,患者应激反应小,咽喉损伤少,清醒后舒适体验好,值得推广使用。
Objective To investigate the clinical efficacy and safety of laryngeal mask general anesthesia in elderly patients undergoing laparoscopic cholecystectomy. Methods From January 2016 to December 2018, 52 patients with laparoscopic cholecystectomy with hy-pertension in our hospital were randomly divided into two groups. In the experimental group, 26 cases were treated with general anes-thesia in the laryngeal mask. In the control group, 26 cases were treated with tracheal intubation. The intraoperative vital signs were compared between the two groups, and the postoperative ventilation related complications and the patient's anesthesia experience sat-isfaction were counted. Results In the control group, HR, SBP and DBP fluctuated significantly at T1 and T4, and the experimental group was relatively stable. The incidence of postoperative ventilation-related complications in the experimental group(7.69%) was lower than that in the control group(26.92%), and the anesthesia experience satisfaction(96.15%) was higher than that in the control group(80.77%). The difference was statistically significant(χ~2=8.316, P<0.05; χ~2=5.739, P<0.05). Conclusion In elderly patients with hypertension, laparoscopic cholecystectomy has a general anesthesia in the laryngeal mask. The patient has less stress response, less throat damage, and a comfortable experience after waking. It is worthy of promotion.
