新辅助化疗联合腹腔镜辅助胃癌根治术治疗进展期胃癌的疗效

摘要
目的分析进展期胃癌者以腹腔镜胃癌根治术配合新辅助化疗治疗的有效性。方法方便选取2014年6月—2017年5月期间医院收治的97例进展期胃癌者为该次观察对象,按数字表法随机分组,对照组48例(行腹腔镜胃癌根治术)、观察组49例(腹腔镜胃癌根治术+新辅助化疗),观察两组手术情况(手术时间、术中出血量、肛门排气),组间比较治疗前后生存质量评分差异,统计手术并发症情况,评估疾病治疗效果。结果观察组治疗有效率为67.3%,高于对照组52.1%,差异有统计学意义(χ2=4.802,P<0.05);观察组术中出血量少,术后住院时间、生存质量评分高(P<0.05);观察组并发症发生率为16.3%,高于对照组6.3%,差异有统计学意义(χ2=4.988,P<0.05)。结论进展期胃癌以新辅助化、腹腔镜胃癌根治术进行治疗,能有效控制肿瘤发展,提高患者生存质量,手术时间短、出血量少,但并发症增加。
Objective To analyze the efficacy of laparoscopic radical gastrectomy combined with neoadjuvant chemotherapy in patients with advanced gastric cancer. Methods 97 patients with advanced gastric cancer admitted to the hospital from June 2014 to May 2017 were convenient selected and randomly divided into groups according to the digital table. 48 patients in the control group(laparoscopic radical gastrectomy) and observation group of 49 patients(laparoscopic radical gastrectomy plus neoadjuvant chemotherapy), the operation of the two groups(surgery time, intraoperative blood loss, anal exhaust), the difference in quality of life scores before and after treatment, statistical complications, the effectiveness of the disease treatment were compared and assessed. Results The effective rate was 67.3% in the observation group and 52.1% in the control group. The difference was statistically significant(χ2=4.802, P<0.05). The amount of bleeding in the observation group was small, and the postoperative hospital stay and quality of life score were high(P<0.05); observed complication rate was 16.3% higher than the control group 6.3%, the difference was statistically significant(χ2=4.988, P<0.05). Conclusion Advanced gastric cancer with neoadjuvant and laparoscopic radical gastrectomy can effectively control tumor development,improve patients' quality of life, shorten operation time, with less bleeding, but the complications are increased.
