2017年芜湖市暗娼人群艾滋病知识、行为及艾滋病与梅毒感染状况调查

摘要
目的了解2017年芜湖市不同档次场所的暗娼人群艾滋病知识的知晓情况、行为特征,艾滋病、梅毒感染状况及暗娼人群在商业性性行为中使用安全套的影响因素。方法采用《全国艾滋病哨点监测方案》中的暗娼人群哨点调查问卷,对芜湖市主城区不同档次场所中暗娼人群进行行为学调查和血清学调查。结果共调查商业性性服务工作者400人,其中低档暗娼146人占36.50%;艾滋病知识知晓率为94.25%,中高档次的暗娼知晓率为96.06%,与低档次的91.10%相比差异有统计学(P<0.05);调查对象HIV抗体检测均为阴性,梅毒感染率为3.75%,低档暗娼梅毒感染率为6.75%,而中高档暗娼梅毒感染率仅为1.97%,两者差异有统计学意义(P<0.05);调查对象最近一次与客人发生性关系时安全套的使用率为97.75%,最近一个月与客人发生性关系时每次都使用安全套的比率为92.50%,不同档次暗娼安全套使用率差异无统计学意义。Logistic回归分析结果显示年龄越大的暗娼,来自洗脚屋/发廊的比来自桑拿/洗浴中心的暗娼,在发生商业性行为时安全套每次使用比例较低;而对艾滋病知识越知晓的暗娼安全套每次使用率越高。结论暗娼人群艾滋病知识知晓率和安全套使用率均较高,感染艾滋病的风险较低,但是需要加强洗脚屋/发廊等低档次暗娼的行为干预工作。
        Objective To understand the status of knowledge and behavior related to AIDS, prevalence of HIV infection and syphilis, and influencing factors of condom use in commercial sex among female sex workers(FSWs) in Wuhu City.Methods A survey was conducted by using the questionnaire on FSWs from National Sentinel Surveillance Program for AIDS to collect the AIDS knowledge and behavioral information and the serological tests were performed among the different ties of FSWs in main areas of Wuhu City.Results Of the total of 400 FSWs, 146 low-tie FSWs were investigated accounting for 36.50%. The total rate of knowledge related to AIDS was 94.25%, and the rates among the middle-tie/high-tie FSWs and low-tie FSWs were 96.06% and 91.10%, respectively, with statistically significant difference(P<0.05). All the participants were negative for the HIV test, and the prevalence of syphilis was 3.75%. The prevalence rates of syphilis among the low-tie FSWs and middle-tie/high-tie FSWs were 6.75% and 1.97%, respectively(P<0.05). The rates of condom use in the last commercial sex and in commercial sex in past month were 97.75% and 92.50%, respectively, without statistically significant difference. The logistic regression analysis showed that the elder, the lower rate of consistent condom use among FSWs. Ones from footbath house/hair salon reported lower rate of consistent condom use in commercial sex than those from sauna/bath centers. FSWs with more knowledge related to AIDS reported the higher rate of consistent condom use.ConclusionThe rates of knowledge related to AIDS and condom use are high and the risk of infecting HIV is low among FSWs in the main areas of Wuhu City.Though, the behavioral intervention still should be enhanced for low-tie FSWs from footbath house/hair salon and other places.
