尖吻蝮蛇血凝酶在甲状腺手术围手术期应用的有效性及安全性观察

摘要
目的:观察尖吻蝮蛇血凝酶在甲状腺围手术期应用的有效性及安全性。方法:60例单侧甲状腺切除的手术患者随机分为试验组及对照组,各30例。在术前15min及术后第12h分别静脉注射尖吻蝮蛇血凝酶2U/次及安慰剂;分别记录术中出血量及术后24h引流量;两组均于术前及术后第1天检测血常规,凝血功能,肝肾功能,计算术前术后各指标的变化。术前及术后第5天分别检测双下肢静脉彩超。结果:试验组术中出血量、术后引流量明显少于对照组,P<0.05;两组术前、术后血常规、凝血功能及肝肾功能各指标变化的差异无统计学意义。入组患者术前及术后双下肢彩超均未见血栓形成。结论:在单侧甲状腺腺叶切除的患者围手术期应用尖吻蝮蛇血凝酶有效且安全。
Objective:Toobserve the effect of hemocoagulase Agkistrodon on the efficacy and safety of perioperative application of thyroid.Methods:60 patients with unilateral thyroidectomy were randomly divided into experimental group and control group,each with 30 cases.In the preoperative 15 min and postoperative 12 h were intravenous injection of hemocoagulase Agkistrodon 2U/and placebo were recorded;the amount of intraoperative bleeding and postoperative drainage volume 24h;two groups were performed preoperatively and first days after the blood routine examination,blood coagulation function,liver and kidney function,change of each index is calculated before and during operation after.Preoperative and postoperative fifth days,respectively,the detection of lower limb venous ultrasound.Results:The amount of bleeding,postoperative drainage volume of experimental group was significantly less than the control group,P<0.05;two groups before surgery,the difference function of blood clotting and liver function indexes change was not statistically significant Before and after surgery,both patients had no thrombosis in the lower limbs.Conclusion:Unilateral thyroid gland resection in patients with perioperative application of hemocoagulase Agkistrodon is safe and effective.
