血乳酸水平测定在急性心肌梗死合并心力衰竭患者救治中的临床意义

摘要
目的:探讨血乳酸水平测定在急性心肌梗死合并心力衰竭患者救治中的临床意义。方法:选取2016年1月～2018年8月期间收治的80例急性心肌梗死合并心力衰竭患者为研究对象,依据血乳酸值水平分为血乳酸升高组(54例)与血乳酸正常组(26例)。结果:血乳酸升高的患者住院期间发生严重心律失常、梗死后综合征、心源性猝死等总并发症的几率明显高于血乳酸正常组,两组比较差异有统计学意义(P <0. 01)。血乳酸升高组患者随访期间再次发生非致命性心肌梗死的几率和心源性猝死的几率均明显高于血乳酸正常组患者,两组比较差异具有统计学意义(P <0. 05)。结论:监测血乳酸水平对于急性心肌梗死合并心力衰竭患者的救治过程具有积极的提示意义,对预测病情严重程度及预后至关重要。
Objective To explore the clinical significance of blood lactate level in the treatment of patients with acute myocardial infarction complicated with heart failure. Method 80 patients with acute myocardial infarction complicated with heart failure admitted to our hospital from January 2016 to August 2018 were selected as the object of study,according to the level of blood lactate,the patients were divided into the elevated group(54 cases) and the normal group(26 cases). Results The patients with elevated blood lactic acid were more likely to have severe arrhythmia,post-infarction syndrome,sudden cardiac death and other total complications during hospitalization than the normal group(P < 0. 01). During the follow-up,the rate of non-fatal myocardial infarction and the rate of sudden cardiac death were significantly higher than that of the normal group. By comparison,the difference was statistically significant(P < 0. 05). Conclusion Monitoring blood lactate level is of positive significance for the treatment of patients with acute myocardial infarction complicated with heart failure,and is of vital importance for the prediction of severity and prognosis of the disease.
