双侧开窗减压与全椎板切除内固定治疗老年退变性椎管狭窄的疗效对比

摘要
目的对比双侧开窗减压与全椎板切除内固定治疗老年退变性椎管狭窄患者的效果。方法选取我院2015年1月至2018年2月收治的100例老年退变性椎管狭窄患者,随机分为A组与B组各50例。A组采用双侧开窗减压内固定术, B组采用全椎板切除内固定术。比较两组患者的治疗效果、椎管直径、 VAS评分及ADL评分。结果两组患者的总有效率比较无统计学差异(P>0.05)。A组的椎管直径显著长于B组(P <0.05), VAS评分、 ADL评分显著低于B组(P <0.05)。结论双侧开窗减压与全椎板切除内固定治疗老年退变性椎管狭窄患者的效果均较好,但双侧开窗减压内固定术能够显著增加椎管直径,改善术后疼痛及生活质量,值得应用与推广。
Objective To compare the effects of bilateral open window decompression internal fixation and total laminectomy internal fixation in the treatment of elderly patients with degenerative vertebral canal stenosis. Methods 100 cases of elderly patients with degenerative vertebral canal stenosis admitted to our hospital from January 2015 to February 2018 were selected and randomly divided into group A and group B, with 50 cases in each group. Group A received bilateral open window decompression internal fixation, while group B received total laminectomy internal fixation. The treatment effect, vertebral canal diameter, VAS score and ADL score were compared between the two groups. Results No statistical difference was found in the total effective rate between the two groups(P >0.05). The spinal vertebral diameter of group A was significantly longer than that of group B(P <0.05), and the VAS score and ADL score were significantly lower than those of group B(P <0.05). Conclusions Bilateral open window decompression internal fixation and total laminectomy internal fixation in the treatment of elderly patients with degenerative vertebral canal stenosis have better treatment effect, but the bilateral open window decompression internal fixation can significantly improve the vertebral canal diameter, and improve the postoperative pain and quality of life, which is worthy of application and promotion.
