玻璃体切除联合黄斑前膜剥除术治疗特发性黄斑前膜疗效观察

摘要
目的探讨特发性黄斑前膜(IMEM)行玻璃体切除联合黄斑前膜剥除术治疗的效果。方法选择2016年2月至2017年1月收治住院的IMEM患者34例(34只眼),采用玻璃体切除联合黄斑前膜剥除术。观察术前、术后1、3个月术眼的矫正视力、黄斑中心凹厚度、对比敏感度及视物变形程度手术前后变化,并进行统计学分析。结果术后1、3个月矫正视力较术前显著提高(P<0.05)。术后1、3个月黄斑中心凹厚度较术前显著减少(P<0.05),术后3个月黄斑中心凹厚度较术后1个月显著减少(P<0.05)。术后1、3个月对比敏感度各频率较术前均有显著提高(P<0.05)。术后1、3个月视物变形程度较术前显著减轻(P<0.05)。结论 IMEM行玻璃体切除联合前膜剥除术治疗可提高患者视力及对比敏感度,减轻黄斑水肿及改善视物变形,手术疗效确切,并发症少,是一种安全、有效的治疗方法。对比敏感度检测可用于评价IMEM手术治疗效果。
Objective To explore the therapeutic effects of combined vitrectomy and membrane peeling for idiopathic macular membrane. Methods Data of 34 patients(34 eyes) with idiopathic macular membrane were collected in Hebei Eye Hospital from February 2016 to January 2017. All patients underwent vitrectomy and membrane peeling. Corrected visual acuity,macular fovea thickness,contrast sensitivity and degree of visual deformation were recorded before surgery,1 month and 3 months after surgery,and statistical analysis was conducted. Results Corrected visual acuity at 1 month and 3 months after surgery improved markedly. The thickness of macular fovea decreased significantly. Contrast sensitivity was significantly higher than before surgery( P < 0. 05). Degree of visual deformation significantly reduced at 1 month and 3 months after surgery( P < 0. 05). Conclusions Combined vitrectomy and membrane peeling improved the patients' visual acuity and contrast sensitivity,relieved macular edema and improved the visual distortion. Contrast sensitivity can be used to evaluate the effects of idiopathic macular membrane.
