医院放射人员染色体畸变与累积辐射剂量的关系研究

摘要
目的探讨医院放射人员染色体畸变与累积辐射剂量的关系。方法选取唐山市各级医院放射人员1 217例,行染色体检查和累积辐射剂量监测,根据工种、工龄、医院级别进行分组。选取具有可比性的非放射人员170例为对照组,行染色体检查。分析染色体畸变与累积辐射剂量的关系。结果工种间比较,依对照组、放射诊断组、介入治疗组、放射治疗组、核医学组顺序,染色体畸变率(dic畸变率、ace畸变率、染色单体型畸变率、合计)呈增长趋势;依放射诊断组、介入治疗组、放射治疗组、核医学组顺序,累积辐射剂量呈增长趋势(P<0.05)。工龄间比较,依对照组、≤10年、10<~≤20年、20<~≤30年、30<年顺序,染色体畸变率(dic畸变率、ace畸变率、染色单体型畸变率、合计)呈增长趋势;依≤10年、10<~≤20年、20<~≤30年、30<年顺序,累积辐射剂量呈增长趋势(P<0.05)。医院级别间比较,依对照组、二甲以下、三甲、二甲顺序,染色体畸变率(dic畸变率、ace畸变率、染色单体型畸变率、合计)呈增长趋势;依二甲以下、三甲、二甲顺序,累积辐射剂量呈增长趋势(P<0.05)。染色体畸变与累积辐射剂量呈明显正相关性。结论工龄越长,累积辐射剂量越大,染色体畸变率越高。医院放射人员应该注重防护工作。
        Objective To investigate the correlation between chromosome aberration and cumulative radiation dose in hospital radiation personnel.Methods The 1217 hospital radiation personnel who were selected from Tangshan hospitals and received chromosome examination and cumulative radiation dose monitoring,were divided into groups according to work type,working years,hospital level.Moreover 170 comparable non radiation workers were enrolled as control group,who received chromosome examination.The correlation between chromosome aberration and cumulative radiation dose was analyzed.Results According to comparison among different work types,the chromosome aberration rates(dic aberration rate,ace aberration rate,chromatid aberration rate,total aberration rate) were increased in order of control group,radiation diagnosis group,interventional treatment group,radiotherapy group,nuclear medicine group,however,the cumulative radiation dose showed an increasing trend in order of radiation diagnosis group,interventional treatment group,-radiotherapy group,nuclear medicine group(P < 0.05).According to comparison among different working years,the chromosome aberration rates(dic aberration rate,ace aberration rate,chromatid aberration rate,total aberration rate) showed an increasing trend in order of control group,≤10 years group,10 < ~ ≤20 years group,20 < ~ ≤30 years group,30 < years group,while the cumulative radiation dose was increased in order of ≤10 years group,10 < ~ ≤20 years group,20 < ~ ≤30 years group,30 < years group(P <0.05).According to comparison among different hospital levels,the chromosome aberration rate(dic aberration rate,ace aberration rate,chromatid aberration rate,total) showed an increasing trend in order of control group,below the second-grade class-A hospitals,the third-grade class-A hospitals,the second-grade class-A hospitals,however,cumulative radiation dose showed an increasing trend in order of below the second-grade class-A hospitals,the third-grade class-A hospitals,the second-grade class-A hospitals(P < 0.05).Conclusion The longer working years,the bigger cumulative radiation dose,the higher chromosome aberration rate is,therefor,hospital radiation personnel should pay much attention to protective measure.
