慢性牙周炎患者社会心理问题调查及其与疾病严重程度的关系

摘要
目的:调查慢性牙周炎患者社会心理问题,并分析其与疾病严重程度的相关性。方法:对220例慢性牙周炎患者(观察组,其中轻度66例,中度93例,重度61例)、100例健康人员(对照组)行牙周检查、焦虑自评量表(SAS)、抑郁自评量表(SDS)及生活事件量表(LES)评定,偏相关分析SAS、SDS、LES与牙周袋深度(PD)、牙石指数(CI)、牙周附着丧失(CAL)的关系。结果:各组PD、CI、CAL、SAS评分、SDS评分、负性生活刺激量、生活事件总刺激量比较差异均有统计学意义(F=23.685,8.580,38.697,12.320,11.462,13.671,10.734;P<0.05);偏相关性分析显示,SAS、SDS、LES评分均与CAL正相关(r=0.433,0.481,0.514;P<0.05),SDS评分与CI正相关(r=0.450,P<0.05)。结论:慢性牙周炎患者易出现焦虑、抑郁、负性生活事件等社会心理问题,且其发生与牙周炎严重程度有关。
Objective:To investigate psychosocial problems in patients with chronic periodontitis,and analyze the correlation with the severity of the disease.Methods:A total of 220 patients with chronic periodontitis(observation group,including 66 mild cases,93 moderate cases and 61 severe cases)and 100 healthy persons(control group)were given periodontal examination and were evaluated with the Self-rating Anxiety Scale(SAS),the Self-rating Depression Scale(SDS)and the Life Events Scale(LES).Partial correlation analysis was performed to analyze the correlation of SAS,SDS and LES with the periodontal pocket depth(PD),calculus index(CI)and clinical attachment loss(CAL).Results:There were significant differences between groups in PD,CI,CAL,SAS score,SDS score,negative life stimulating quantity and the total life event stimulating quantity(F=23.685,8.580,38.697,12.320,11.462,13.671,10.734;P<0.05).Partial correlation analysis showed that scores of SAS,SDS and LES were positively correlated with CAL(r=0.433,0.481,0.514;P<0.05),and the SDS score was positively correlated with CI(r=0.450,P<0.05).Conclusion:Patients with chronic periodontitis are prone to having anxiety,depression,negative life events and other psychosocial problems,which is related to the occurrence and severity of periodontitis.
