结直肠癌患者健康效用值测量及其影响因素研究

摘要
目的:基于EQ-5D-3L量表测量结直肠癌患者健康效用值,并探讨其影响因素。方法:选择黑龙江省结直肠癌患者收治最为集中的三家医疗机构进行现场调查,采用Kruskal-Wallis检验和Tobit模型分别进行单因素和多因素分析。结果:最终有效应答共300例患者,健康效用值平均值为0.590±0.310。患者在行动、自我照顾、日常活动、疼痛/不适和焦虑/抑郁5个维度存在问题的比例分别为50.0%、54.0%、55.7%、57.0%和57.3%。家庭年收入、治疗方案、疾病分期和是否进行造瘘等具有统计学意义(P<0.01)。结论:结直肠癌患者健康效用值较低,家庭年收入、治疗方案、疾病分期和造瘘是主要影响因素。
Objective: To assess the health utility value of colcorectal cancer(CRC) patients and its determinants based on EQ-5 D-3 L scale. Methods: It selected 3 medical institutions with the most concentration of CRC patients in Heilongjiang. Single factor and multi-factor analysis were conducted based on K-W test and Tobit model. Results: 300 patients were included, with average health utility value as 0.590 ±0.310. Problems in mobility, self-care, usual activities, pain/discomfort and anxiety/depression were50.0%, 54.0%, 55.7%, 57.0% and 57.3%, respectively. Family income, treatment regimen, disease staging and fistula had significant impact on health utility value. Conclusion: The health utility value of CRC patients was poor. Family income, treatment plan, disease staging and fistula were main influencing factors.
