调强放疗联合化疗治疗局部晚期非小细胞肺癌临床疗效分析

摘要
目的评价调强放疗联合化疗治疗局部晚期非小细胞肺癌的临床疗效和不良反应。方法入组2010年1月至2016年12月我科收治的局部晚期非小细胞肺癌患者70例,采用同期对照研究分为研究组(调强放疗联合化疗)和对照组(三维适形放疗联合化疗)。放疗结束后2个月评估疗效和不良反应。结果 70例患者均可评价疗效,研究组总有效率为88.6%,中位生存时间为18.34个月,1 a生存率为77.5%;对照组总有效率为65.7%,中位生存时间为13.98个月,1 a生存率为52.5%,差异均有统计学意义(P均<0.05)。研究组放射性肺炎、放射性食管炎发生率均显著低于对照组,差异均有统计学意义(P均<0.05)。结论调强放疗联合化疗治疗局部晚期非小细胞肺癌优于三维适形放疗联合化疗,而且不会增加不良反应。
Objective To observe the efficacy and toxicities of intensity modulated radiotherapy plus chemotherapy in the treatment of locally advanced non-small cell lung cancer.Methods Seventy patients with locally advanced non-small cell lung cancer were divided into the study group(intensity modulated radiotherapy plus chemotherapy) and the control group(three dimensional conformal radiotherapy plus chemotherapy). The efficacy and toxicities of patients were evaluated after two months of radiotherapy.Results The efficacy of all the 70 patients were evaluated.The response rate was 88.6% in the study group, and was 65.7% in the control group(P<0.05). The median duration of survival were 18.34 and 13.98 months respectively in the two groups(P<0.05). The 1 year survival rates were 77.5% and 52.5% respectively in the two groups(P<0.05).The incidences of radiation pneumonitis and radiation esophagitis in the study group were less than those in the control group(P<0.05).Conclusion Intensity modulated radiotherapy plus chemotherapy is better than three dimensional conformal radiotherapy plus chemotherapy in the treatment of patients with locally advanced non-small cell lung cancer,and the toxicities can be tolerated.
