开窗减压术治疗大型颌骨囊性病变39例临床观察

摘要
目的探讨和评价开窗减压术治疗大型颌骨囊性病变的疗效。方法选择2014年—2017年收治的39例经开窗减压术治疗并随访的大型颌骨囊性病变病例资料进行分析。结果 32例开窗效果良好,囊强体积明显缩小,随访至今未见复发;3例根端囊肿和2例牙源性角化囊性瘤,经开窗治疗后效果不佳;2例单囊型成釉细胞瘤,经开窗治疗并随访4～6个月后,病灶转变为实性成釉细胞瘤且局部生长活跃。结论颌骨牙源性囊肿开窗减压后疗效较满意,而对于牙源性角化囊性瘤或囊性成釉细胞瘤,其疗效还有待进一步观察和评价。
Objective To investigate and evaluate the effect of fenestration decompression in the treatment of large jaw cystic lesions.Methods Totally 39 patients with large cystic lesions of the jaw were treated with fenestration decompression from 2014 to 2017, all the patients were followed up after operation, and all the data was evaluated and analyzed.Results Totally 32 patients had good fenestration effect,the volume of cyst cavity was significantly reduced, and no recurrence was found after follow-up. 3 cases of root cyst and 2 cases of odontogenic keratocystic tumors had poor effect after fenestration treatment. 2 cases of single-cystic ameloblastoma were treated by fenestration and followed up for 4～6 months. The lesion turned into solid ameloblastoma with active local growth.Conclusion The effects of fenestration decompression in the treatment of odontogenic cysts were satisfactory, whereas for the odontogenic keratocystic tumor or cystic ameloblastoma, the curative effect needs further observation and evaluation.
