多模式镇痛在髋关节发育不良中的应用

摘要
目的探究与分析多模式镇痛方案在儿童先天性髋关节发育不良患者术后康复中的临床应用。方法选取我院自2015年3月—2018年3月收治的90例儿童先天性髋关节发育不良患者,采取随机数字表法分为对照组与观察组,每组各45例,对照组仅给予传统药物镇痛方法,观察组在对照组基础上实施多模式镇痛方法,对比两组患儿家属对不同镇痛方案的满意度,并比较不同时间点疼痛评分。结果对照组患儿家属的满意度为75.56%,观察组患儿家属的满意度为93.33%,观察组患儿家属的满意度高于对照组相,差异具有统计学意义(P<0.05)。两组患者术后12 h与术后6 h相比疼痛评分较高,但术后24 h、术后48 h分别与术后12 h相比疼痛评分降低,但仍然高于术后6 h,观察组与对照组相比术后12 h、术后24 h及术后48 h的疼痛评分均较低,差异具有统计学意义(P <0.05)。结论将多模式镇痛方案应用于儿童先天性髋关节发育不良患者中能够获得更高的患儿家属满意度,并有效的降低并缓解术后疼痛,安全性更高。
Objective To explore and analyze the clinical application of multimode analgesic regimen in the postoperative rehabilitation of children with congenital hip dysplasia. Methods A total of 90 children with congenital hip dysplasia who were admitted to our hospital from March 2015 to March 2018 were selected and randomly divided into a control group and an observation group. 45 patients in each group and the control group was only given traditional medicine analgesic method. The observation group was treated with oral ibuprofen and incision continued ice analgesia on the basis of the control group. The degree of satisfaction of the nursing care program was compared between the two groups of children and the pain scores were compared at different time points. Results The care satisfaction of the family members of the control group was 75.56%. The satisfaction degree of the family members of the observation group was 93.33%. The satisfaction degree of the observation group was higher than that in the control group and the difference had a statistically significant(P < 0.05). There was higher pain scores postoperatively in two groups at 12 h and 6 h, but the pain scores was reduced at 24 h postoperatively and 48 h postoperatively compared with that at 12 h postoperatively, but still higher than postoperative 6 h, but the pain scores of the observation group and control group were lower than those at 12 h postoperatively, 24 h postoperatively, and 48 h postoperatively, the difference had a statistically significant. Conclusion The application of multi-modality analgesia nursing program in children with congenital hip dysplasia can obtain higher degree of family satisfaction in children, and effectively reduce and relieve postoperative pain, with higher safety.
