中国老年衰弱患病率的系统评价

摘要
目的系统评价中国人群老年衰弱的患病率。方法计算机检索PubMed、EMbase、Web of Science、CNKI、VIP和WanFang Data数据库,搜集关于中国老年衰弱患病率的研究,检索时限均从1978年1月至2018年12月。由2名研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用OpenMetaAnalyst软件进行Meta分析。结果共纳入29个研究,包括64 306例患者。Meta分析结果显示:中国老年社区人群衰弱患病率为12.8%[95%CI(0.105,0.152)],医院人群衰弱患病率为22.6%[95%CI(0.130,0.270)],养老机构人群衰弱患病率为44.3%[95%CI(0.222,0.664)]。按性别进行亚组分析:社区男性衰弱患病率为9.2%[95%CI(0.074,0.110)],女性衰弱患病率为12.9%[95%CI(0.100,0.157)]。医院男性衰弱患病率为23.3%[95%CI(0.124,0.342)],医院女性衰弱患病率为17.9%[95%CI(0.122,0.237)]。按地区进行亚组分析:华北地区社区患病率为13.4%[95%CI(0.093,0.175)],华东地区社区患病率为13.6%[95%CI(0.083,0.189)],台湾、香港地区社区患病率为9.8%[95%CI(0.078,0.199)]。按年龄分层:65~74岁老人衰弱患病率为12.2%[95%CI(0.037,0.206)],75~84岁患病率为33.2%[95%CI(0.136,0.527)],85岁及以上患病率为46.8%[95%CI(0.117,0.820)]。按调查时段分层:2003~2010年社区人群衰弱患病率为9.1%[95%CI(0.078,0.104)],2011~2018年社区人群患病率为14.4%[95%CI(0.111,0.177)]。结论中国老年衰弱患病率较高,经济欠发达地区较发达地区高,医院人群衰弱患病率高于社区人群,社区、农村地区女性患病率较男性高,年龄越大患病率越高,2011年以后患病率较以前更高。因无统一筛查工具,本研究结果尚需更多高质量研究予以验证。
        Objectives To systematically review the epidemiological characteristics of frailty in Chinese elderly population. Methods PubMed, EMbase, Web of Science, CNKI, VIP and WanFang Data databases were electronically searched to collect studies on the epidemiological characteristics of frailty in China from January, 1978 to December, 2018.Two reviewers independently screened literature, extracted data and assessed risk of bias of included studies, and then,meta-analysis was performed by using OpenMetaanalyst software. Results A total of 29 studies, involving 64 306 patients were included. The results of meta-analysis showed that: the prevalence of frailty in Chinese community was 12.8%(95%CI 0.105 to 0.152), in Chinese hospital was 22.6%(95%CI 0.130 to 0.270), and in nursing home was 44.3%(95% CI0.222 to 0.664). In the subgroup analysis by gender: male prevalence in community was 9.2%(95%CI 0.074 to 0.110),female was 12.9%(95%CI 0.100 to 0.157). Prevalence of male in hospital was 23.3%(95%CI 0.124 to 0.342), Prevalence of female was 17.9%(95%CI 0.122 to 0.237). In the subgroup analysis by region: the prevalence of frailty in north China was13.4%(95%CI 0.093 to 0.175), in east China was 13.6%(95%CI 0.083 to 0.189), and in Taiwan province and HongKong was 9.8%(95% CI 0.078 to 0.199). In the subgroup analysis by age: the prevalence of frailty among the elderly aged 65 to74 was 12.2%(95%CI 0.037 to 0.206), and 33.2%(95%CI 0.136 to 0.527) among the elderly aged 75 to 84, and 46.8%(95%CI 0.117 to 0.820) among the elderly aged above 85. For investigation based on time periods, the prevalence of frailty was 9.1%(95%CI 0.078 to 0.104) from 2003 to 2010, and 14.4%(95%CI 0.111 to 0.177) from 2011 to 2018. Conclusions The prevalence of frailty in China is high. Economic underdevelopment is higher in more developed regions. The prevalence in hospital population is higher than that in community population. The prevalence of female in community and rural is higher than male. The higher ages has higher prevalence. The prevalence of frailty from 2011 to 2018 is higher than from2003 to 2010. As no uniform screening tools exist, the results of this study requires further validations by high quality research.
