直接全冠修复老年隐裂磨牙的临床疗效分析

摘要
目的研究直接全冠修复老年隐裂磨牙的临床疗效。方法选择2015年1月至2017年12月在兵器工业部卫生研究所521医院和解放军第三医院口腔科首诊且未进行牙髓治疗的隐裂磨牙患者120例(120颗牙),均符合条件可进行直接全冠修复处理。随访1～6个月进行疗效判断。结果随访1～6个月,120例老年患者全冠修复成功61例(50. 8%),改善11例(9. 2%),失败48例(40. 0%),总有效率为60. 0%。治疗失败的48例患者均在全冠修复1～2个月后发生牙髓炎或根尖周炎,其中12例在X线下出现根尖周暗影,经过牙髓治疗2周后症状消失,然后进行后续治疗。结论老年发生隐裂牙经评估可以进行全冠修复时应尽早处理,以提高患者的生活质量。
Objective To study the curative effect of the direct crown restoration for cracked molar in elderly patients.Methods From Jan. 2015 to Dec. 2017,first visit in the Stomatology Department of 521 Hospital,Institute of Health,Ministry of Weapons Industry and PLA Third Hospital,and no dental pulp therapy,120 patients( 120 teeth) with cracked molars,were included in the study. All of these patients were eligible for direct crown restoration. After 1-6 months followup,the curative effect was evaluated. Results The follow-up of 1 to 6 months showed that 61 cases( 50. 8%) of the 120 elderly patients were successful in crown restoration,11 cases( 9. 2%) were improved and 48 cases( 40. 0%) were failed.The total effective rate was 60. 0%. 48 cases of failed treatment developed pulpitis or periapical periodontitis 1-2 months after crown restoration,12 of which had periapical shadow under X-ray,symptoms disappeared after 2 weeks of pulp treatment,and then received following treatment. Conclusion The elderly patients with cracked molars can be treated with crown restoration as early as possible to improve their quality of life.
