青海省三江源地区布鲁杆菌病感染的相关因素分析

摘要
目的了解青海省三江源地区2012-2013年布鲁杆菌病(简称布病)的感染情况,分析引起三江源地区布病感染的可能相关因素,为制定符合该地区布病的防治措施提供依据和建议。方法按照多阶段分层随机抽样方法对随机抽取的人群进行布病流行病学调查,采用病例对照研究方法分析三江源地区布病感染的情况,多因素非条件Logistic回归方法分析布病感染的相关因素。结果调查6 310人,血检6 310人,布病感染率2.24%、患病率0.523%、发病率1.18/万,不同性别(χ2=18.931,P<0.05)、年龄(χ2=36.151,P<0.05)、职业(χ2=23.510,P<0.05)、民族(χ2=13.322,P<0.05)的布病感染差异有统计学意义。与三江源地区布病感染相关的因素有:接触史、人羔同居一室、流产物的无处理、接触病畜、接触病畜时无自我防护、食用煮不熟的肉、饲养家畜、食用生奶。多因素非条件Logistic回归分析显示,布病感染的危险因素有接触史、人羔同居一室、流产物的无处理、接触病畜、接触病畜时无自我防护、食用煮不熟的肉、食用生奶,布病感染的保护因素有饲养家畜。结论青海省三江源地区布病感染与多种相关行为因素有关,应加强其防治工作。
Objective To understand the status of the brucellosis infection in San jiangyuan region of Qinghai province during 2012-2013,analyzed the possible related factors which cause brucellosis among Sanjiangyuan region,to provide references and suggestions for the establishing prevention measures of brucellosis in this area. Methods Conduct brucellosis epidemiological survey on a random sample of population according to the multi stage stratified random sampling method,analyze brucellosis infection situation in Sanjiangyuan region by the method of case-control study,analyzed brucellosis infection related factors by the method of multiple factors non-conditional logistic regression. Results Surveyed of 6310 people,blood examination of 6310 people,brucellosis infection rate was 2. 24%,prevalence rate was 0. 523%,incidence of a disease was 1. 18 /10000,there was had statistical significance difference in gender( χ2=18. 931,p <0. 05),age( χ2=36. 151,p < 0. 05),occupation( χ2= 23. 510,p < 0. 05),nationality( χ2= 13. 322,p<0. 05) of the brucellosis infection. Factors associated with the infection of brucellosis in Sanjiangyuan region include:contact history,people and lamb live in one room,the lambs abortion without treatment,contact with patient animals,contact with infected animals without self protection,edible uncooked meat,livestock production,and consumption of raw milk. Non conditional logistic regression analysis showed include: contact history,people and lamb live in one room,the lambs abortion without treatment,contact with patient animals,contact with infected animals without self protection,edible uncooked meat,and consumption of raw milk,brucellosis infection risk factor was livestock production. Conclusions Brucellosis infection is related to multiple relevant behavior sources in Sanjiangyuan region of Qinghai province,should strengthen the prevention and control work.
