凉血清热方治疗寻常型银屑病血热证40例疗效观察

摘要
目的观察凉血清热方治疗寻常型银屑病血热证的临床疗效。方法将80例寻常型银屑病血热证患者随机分为治疗组40例、对照组40例。治疗组予凉血清热方口服,对照组予复方青黛胶囊口服,2组均外用芩柏膏。观察2组中医证候疗效、中医症状评分、银屑病面积和严重程度指数(PASI)评分情况。结果中医证候疗效治疗组40例中,显效33例(82.5%),有效7例(17.5%),无效0例(0.0%),总有效40例,总有效率为100.0%;对照组40例中,显效16例(40.0%),有效18例(45.0%),无效6例(15.0%),总有效34例,总有效率为85.0%。经秩和检验,Z=-2.371,P<0.05,说明治疗组中医证候疗效优于对照组。2组治疗后红斑、鳞屑、瘙痒、口干咽痛、心烦易怒、大便方面评分较本组治疗前均降低(P<0.05)。治疗组各项评分较对照组降低明显,治疗前后评分差值与对照组比较均有统计学意义(P<0.05),说明治疗组优于对照组。2组PASI评分治疗后较本组治疗前均降低(P<0.05),治疗组较对照组降低明显,2组前后差值比较差异有统计学意义(P<0.05),说明治疗组优于对照组。结论凉血清热方治疗寻常型银屑病血热证具有较好的临床疗效。
Objective To observe the curative effect of Liangxue Qingre decoction on psoriasis vulgaris(syndrome of blood heat). Methods 80 patients with psoriasis vulgaris(syndrome of blood heat) were randomly divided into control group( n = 40) and treatment group( n = 40). The treatment group was given Liangxue Qingre decoction,while the control group was treated with capsule of compound Qingdai,both groups were treated with Qinbai cream. Traditional Chinese medicine syndromes,changes of symptom integrals,psorasis area and severity index integrals( PASI) were observed between two groups. Results In treatment group,there were 33 cases(82. 5%) showed obvious effect,7(17. 5%) effective,no case ineffective,and the total effective rate was 100. 0%. In control group,there were 16 cases(40. 0%)showed obvious effect,18( 45. 0%) effective,6( 15. 0%) ineffective,and the total effective rate was85. 0%. Rank-sum test showed Z =-2. 371,P < 0. 05,which indicated Chinese medicine pattern of treatment group was superior to the control group. After treatment,the scores of erythema,scales,itching,dry mouth and pharyngeal pain,upset and easy to be angry,defecation decreased in both groups( P <0. 05). The scores of the treatment group decreased significantly than control group,and the differential before-after treatment was statistically different than the control group( P < 0. 05),which indicated the treatment group was superior to the control group. After treatment,PASI decreased significantly in both groups( P < 0. 05),and it decreased more in the treatment group,the differential before-after treatment in both groups were statistically different( P < 0. 05),which indicated the treatment group was superior to the control group. Conclusion Liangxue Qingre decoction has good effect on psoriasis with pattem of blood heat.
