摘要
目的:探讨负重步行对心功能Ⅰ—Ⅱ级患者心肌耗氧量的影响。方法:将54例心功能Ⅰ—Ⅱ级患者进行自身-前后对照研究,第1天步行设为对照组,第2天负重5kg步行(模拟买菜)设为观察组,观测两组不同时段心率、收缩压和心肌耗氧量情况。结果:对照组和观察组T_1到T_5时段心率、收缩压和心肌耗氧量增加;在T_2、T_3、T_4、T_5时段,负重步行组心率和心肌耗氧量均高于对照组,且在T_4时段到达高峰,差异均有统计学意义(均P<0.05);在T_3和T_4时段,负重步行组收缩压高于对照组,且在T_4时段达高峰,差异有统计学意义(P<0.05);负重步行组各时段的心肌耗氧量两两比较显示,T_0与T_2—T_5、T_1与T_3—T_5及T_2与T_3—T_5比较差异均有统计学意义(均P<0.05)。结论:可根据心肌耗氧量调整心功能Ⅰ—Ⅱ级患者的活动强度。
        Objective:To investigate the effects of weight-bearing walking on myocardial oxygen consumption in patients with grade Ⅰ—Ⅱ cardiac function.Methods:Fifty-four patients with grade Ⅰ—Ⅱ cardiac function were conducted a self-control study,walking on the first day was regarded as the control group and walking with a weight of 5 kg(simulating green grocery shopping)on the second day was regarded as the experimental group.Then the heart rate,systolic pressure and myocardial oxygen consumption were recorded at different time points.Results:During T_1 to T_5,heart rate,systolic pressure and myocardial oxygen consumption were increased both in the control group and the experimental group.The heart rate and myocardial oxygen consumption in T_2,T_3,T_4 and T_5,the systolic blood pressure in T_3 and T_4 were significant higher in the experimental group than those in control group(All P<0.05),with the peak value at T_4.Pairwise comparison of myocardial oxygen consumption in different periods time of weight-bearing walking was carried out,and the difference were statistically significant between T_0 and T_2—T_5,T_1 and T_3—T_5,T_2 and T_3—T_5(All P <0.05).Conclusion:The activity intensity of patients with grade Ⅰ—Ⅱcardiac function can be adjusted according to the myocardial oxygen consumption.
