彩色多普勒超声对肝硬化上消化道出血门静脉血流动力学改变的诊断价值

摘要
目的采用彩色多普勒对肝硬化门静脉高压患者门静脉血流进行定量并探讨其与上消化道出血的关系。方法选取来我院进行治疗的肝硬化门静脉高压症患者76例,根据有无出血史进行分组:出血组(n=35)及无出血组(n=41),对照组患者为来我院进行健康体检的38例健康的研究对象,所有研究对象的门静脉血流量的测定采用彩色多普勒进行。结果肝硬化出血组与未出血组与对照组比较具有明显较高的门静脉血流量,较慢的血流速度,并且具有较高的门静脉侧支分流指数,差异具有显著性(P<0.05)。结论肝硬化上消化道出血的预测可以通过彩色多普勒对门静脉血流量的分析完成,可以作为一种有价值的预测手段在临床推广应用。
Objective To analyze the vein blood flow in patients with cirrhosis and portal hypertension with Color Doppler portal and explore its relationship with upper gastrointestinal bleeding.Methods A total of 76 cases with cirrhosis and portal hypertension were collected from our hospital,they were grouped according to the history of bleeding: bleeding group(n = 35) and non-bleeding group(n = 41),control group included 38 healthy subjects who were selected from our hospital,We used color Doppler to measure the portal blood flow of all the objects.Results Cirrhosis hemorrhage group compared with non-bleeding group and control group,the portal blood flow of the bleeding group was higher,slower blood flow velocity was slower,and index of portal venous collaterals shunt was higher,the differences were significant(P < 0.05).Conclusion Of upper gastrointestinal bleeding in the conclusions of cirrhosis forecast by color Doppler door venous flow analysis completed,can be used as a valuable means of prediction in the clinical appli cation.
