810例烧伤患者的成分输血分析

摘要
目的:分析2011~2017年本院烧伤患者的血液成分使用情况,为临床科室进一步科学合理用血提供理论依据。方法:回顾性分析2011~2017年本院810例烧伤患者输注血液成分的种类及使用情况。结果:810例烧伤患者共输注血液成分8 247.12 U,其中红细胞1 656.8 U(20.09%),血浆6 540.62U(79.31%),血小板21.5 U(0.26%),冷沉淀22.2 U(0.27%),全血6.0U(0.07%)。O型血患者成分血用量最多(2 848.08 U,34.53%),AB型血患者用量最少(766.32U,9.29%)。输血患者中,0~2岁婴幼儿患者最多(291例,35.93%)。结论:烧伤患者以无自主安全意识的婴幼儿和从事社会高危行业的中青年为主,使用的血液成分主要是冰冻血浆。临床医生应根据患者病情,及时准确地把握输血时机,科学合理地决定输血成分,在保证烧伤患者精准救治的同时,尽量降低经输血传播疾病的风险。
        Objective:To analyze status of utilization of blood components in the burned patients so as to provide references for scientific and rational blood transfusion during therapy.Methods:A total of 810 cases of burned patients in our hospital from 2011 to 2017 were retrospectively analyzed,including types and amount of component blood transfusion.Results:The total blood components transfused were 8 247.12 U,including red blood cells1 656.8 U(accounted for 20.09%),plasma 6 540.62 U(accounted for 79.31%),platelet 21.5 U(accounted for 0.26%),cold precipitation 22.2 U(accounted for 0.27%),whole blood 6.0 U(accounted for0.07%).O-type blood was the maximum proportion(2 848.08 U,34.53%),AB-type blood was the minimum amount(766.32 U,9.29%).The most amount were found in burned patients during the age of 0-2(n=291,35.93%).Conclusion:The burn patients are mainly infants who have no self-conscious safety consciousness and young persons who engaged in the high risk industry of the society.The main blood components used are frozen plasma.Clinicians should accurately choose transfusion therapy according to the patient′s condition,scientifically and rationally decide the components of blood transfusion,and ensure the precise treatment of burn patients while minimizing the risk of transfusion transmitted diseases.
