两种形态全口义齿对老年无牙患者颞下颌关节紊乱病的影响

摘要
目的:比较两种形态全口义齿对老年无牙患者颞下颌关节紊乱病的影响。方法:选取2016年3月-2018年9月在笔者所在医院进行全口义齿修复的老年无牙患者颞下颌关节紊乱病78例,按照随机数字表法将患者随机分为对照组、试验组,每组39例。对照组患者进行解剖型全口义齿修复,观察组患者进行舌侧集中型全口义齿修复。观察对比两组患者的修复结果。结果:修复后3个月,试验组的功能障碍指数为(0.118±0.040),与修复前(0.238±0.039)相比显著降低,差异有统计学意义(P<0.05)。修复后6个月,试验组的功能障碍指数为(0.079±0.029),与修复后3个月相比,仍有小幅降低,差异有统计学意义(P<0.05)。对照组修复后3、6个月的功能障碍指数分别为(0.161±0.039)和(0.131±0.031),与修复前(0.241±0.041)相比降低,差异有统计学意义(P<0.05)。试验组修复后3、6个月的下降幅度显著优于对照组,差异有统计学意义(P<0.05)。试验组的治疗总满意度为92.31%(36/39),显著高于对照组的76.92%(30/39),差异有统计学意义(P<0.05)。结论:应用舌侧集中型全口义齿修复治疗老年无牙患者颞下颌关节紊乱病效果更佳,使患者的咀嚼功能得以恢复,改善临床症状,有较高的临床应用价值。
Objective:To compare the effects of two types of complete dentures on temporomandibular joint disorders in elderly edentulous patients.Method:From March 2016 to September 2018,78 elderly edentulous patients with temporomandibular joint disorders who underwent complete denture restoration in our hospital were randomly divided into control group and experimental group with 39 cases in each group.Among them,the patients in the control group were repaired with anatomical complete dentures,while those in the observation group were repaired with lingual concentrated complete dentures.The repair results of the two groups were observed and compared.Result:Three months after repair,the dysfunction index of the experimental group was(0.118±0.040),which was significantly lower than(0.238±0.039) of the control group,and the difference was statistically significant(P<0.05).At 6 months after repair,the dysfunction index of the experimental group was(0.079±0.029).Compared with the three months after repair,the dysfunction index of the experimental group was still slightly lower,the difference was statistically significant(P<0.05).The functional impairment indexes of the control group at 3 months and 6 months after repair were(0.161±0.039)and(0.131±0.031),respectively.Compared with(0.241±0.041) before repair,the functional impairment index of the control group decreased significantly,the differences were statistically significant(P<0.05).And at 3 months and 6 months after repair,the decline of the experimental group were significantly higher than those of the control group,the differences were statistically significant(P<0.05).The total treatment satisfaction of the experimental group was 92.31%(36/39),which was significantly higher than 76.92%(30/39) of the control group,the difference was statistically significant(P<0.05).Conclusion:The application of lingual centralized complete denture in the treatment of temporomandibular joint disorders in elderly edentulous patients is more effective,which can restore the masticatory function of patients and improve clinical symptoms,and has a high clinical value.
