硝酸甘油联合小剂量呋塞米治疗高血压急症临床观察

摘要
目的通过硝酸甘油联合小剂量呋塞米治疗高血压急症的临床疗效观察,进一步确定其有效性。方法 50例高血压急症患者,均给予5%葡萄糖250 ml+硝酸甘油15 mg,同时静脉注射呋塞米20 mg。比较患者用药前和用药后15、30、60、90、120 min时血压和心率,观察治疗效果。结果用药后15、30、60、90、120 min,患者收缩压、舒张压均较用药前显著下降,差异具有统计学意义(P<0.05);用药后120 min血压下降明显,92%(46/50)患者平均血压下降15%~25%。用药后15、30、60、90、120 min,患者心率与用药前比较差异无统计学意义(P>0.05);心率波动范围0~37%,且心率在60~100次/min内。50例患者治疗显效30例,有效19例,无效1例,治疗总有效率为98%。结论硝酸甘油联合小剂量呋塞米治疗高血压急症,使用方便,显效较快,疗效可靠,无明显副作用。
Objective To observe the clinical effect of Nitroglycerin combined with low dose Furosemide in the treatment of hypertensive emergencies, in order to further determine its effectiveness. Methods A total of 50 patients with hypertensive emergencies were given 5% glucose 250 ml plus Nitroglycerin 15 mg. Meanwhile, 20 mg of Furosemide was injected intravenously. The blood pressure and heart rate were compared before and 15, 30, 60, 90, 120 min after medication, and the therapeutic effect was observed. Results After 15, 30, 60, 90, 120 min of medication, the systolic blood pressure and diastolic blood pressure of patients were significantly lower than before medication, the differences were statistically significant(P<0.05). After 120 min of medication, the blood pressure decreased significantly, and the average blood pressure of 92%(46/50) patients decreased by 15% to 25%. Compared before medication, there were no significant differences in the heart rate after 15, 30, 60, 90 and 120 min of medication(P>0.05). The range of heart rate fluctuation ranged from 0 to 37%, and the heart rate was 60-100 times/min. In 50 cases, 30 cases were markedly effective, 19 cases were effective, 1 case was ineffective, and the total effective rate was 98%. Conclusion Nitroglycerin combined with low dose Furosemide in the treatment of hypertensive emergencies is convenient to use, take effect faster, has reliable effects and no obvious side effects.
